(Weight Loss & General Surgery Treatments are slightly different).| 1. | Consider which treatment(s) fulfil your needs. |

| 2. | Then fill out the questionnaire. |
| 3. | Upload photos, if required, of the area needing treatment (photos may also be sent by post). |
| 4. | On receiving the questionnaire and photos one of our advisors will contact you and discuss any issues or questions you may have. |
| 5. | We then discuss with the surgeon the results of the questionnaire, the photos and any points arising from the telephone conversation. Once he has assessed the treatment required we will contact you with his recommendations and a fixed price quote. |
| 6. | Given that you wish to accept, all we need initially is for you to book your flight tickets, and forward us a copy, plus your instructions regarding accommodation during your stay. (In most cases we can even book the flight tickets for you). |
| 7. | We will then make the necessary arrangements for your treatments. |
| 8. | Inside of seven days of receiving of this information we will supply you with a copy of any forms needing completion at the clinic, and a concise itinerary detailing your stay in Prague. (We can handle the booking of your accommodation, (should you wish), plus the treatment and transport to and from the clinic, hotel and airport. Payment for the accommodation is made on arrival). |
| 9. | On your approval, we require you to make a deposit payment. The deposit payment can be by Debit/Credit Card, bank transfer, or Cash / Cheque at our bank in the UK. |
| 10. | On your return one of our support team will contact you to monitor your progress. |
